Welcome to Our Online Information Center






We are a comprehensive private practice physical therapy clinic established by Sandra Ladd in 1987. Over the years, the practice has grown to offer expanded services to the Southern Vermont/New Hampshire area.


All patients are evaluated by a licensed Physical Therapist and may be treated in a team environment with a highly trained Physical Therapy Assistant under the appropriate circumstances.


We enjoy close relationships with area physicians and maintain ongoing communication with all of our referral sources.



Our Goal is to be the leader in providing physical therapy services to the people in our community.

If you are you a new patient or returning to see us after more than 6 months since your last visit please take advantage of our

Federal Legislation known as HIPAA went into effect on 4/14/03 and this has increased the amount of paperwork we need you to complete before we can begin our evaluation and treatment so please be sure to click on each of the links below to download the necessary paperwork. The downloads are in PDF format.

This will allow you to maximize your time with your therapist at your first visit!


We have a new option for doing your intake documents.

You can now fill out our paper work on your computer!

You can either:

#1. email us the data from your completed form for us to print here for you and you can sign them when you arrive for your 1st visit (CLICK ON THE SUBMIT BUTTON AT THE END OF THE DOCUMENT)

OR

#2. print out your completed forms at home and bring them with you.
